Before you use an affidavit

An affidavit is a sworn statement: saying something false in it is an offence.
It is usually printed on stamp paper of the value your state requires and signed in front of a notary or an oath commissioner, who attests it. This page does neither.
Many offices publish their own wording for a particular purpose. Use theirs when they do.

Write each fact as its own short statement, in the first person. This is a draft to take to the notary, not legal advice.
